JOB DESCRIPTION

Sanctum Care Group is a growing organization looking for motivated persons to join an innovative organization which is committed to delivering high quality, trauma informed care in a home setting.
Main Activities
Provide supportive care to people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psychosocial factors. Provide Care to women who are pregnant and their newborn children who are also affected by above factors.
Provide supportive care through a patient centered model aimed at creating independence and autonomy while fostering an environment of acceptance and knowledge
Personal care to patients including assisting with grooming, bathing, personal hygiene, toileting, wound care, nutrition, infant care and other activities of everyday living.
Assist with the transition plans from Sanctum and Sanctum 1.5 into the community. Follow medical instructions carefully and consistently.
Assist with meal preparation and planning, household tasks. Manage any unanticipated events or unreliable situations
Qualifications
The prosperous candidates will have the following expertise and practice
Cultural sensitivity of First Nations issues
capability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
availability to adapt to new concepts, techniques and best practices in the academic and clinical fields of chronic disease management, addictions harm reduction and trauma informed care
An knowledge of the issues related to HIV including harm reduction
Conflict resolution and interpersonal expertise
Background working in a health care setting, child / youth care settings or in Mental Health and Addictions
Degree/diploma or certificate in social work, care aid, addictions or related field
CPR/First Aid/Food safety certificate/Knowledge of TLR, up to date immunizations
